On date and time listed, Licensing Program Analyst (LPA) Perla Ordonez arrived at the facility to conduct an annual inspection as part of a compliance review. LPA toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed: Normal days and hours of operation are: Monday \342\200\223 Sunday; 23 hours a day. OFF-LIMIT AREAS INCLUDE: Bedroom 1, bedroom 2, bedroom 3, laundry room, garage, and courtyard. The facility is operating within the licensed capacity and appropriate ratios. \302\267 Appropriate supervision provided during this inspection. \302\267 A working telephone is present and current number on file. \302\267 Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection. \302\267 Fireplace is properly screened to prevent access by children. \302\267 All hazardous items are not stored inaccessible to children. During facility tour, LPA observed that fencing in the on-limits backyard that bordered the Courtyard was in disrepair as the wood was displaced, had splintered edges, and had nails/staples which stuck out. Children present were asleep at the time of the visit and Licensee stated that children had yet to go outside today but that they were to go outside later in the day. \302\267 Toxins are not locked. During facility tour, LPA observed a poisons/toxin in the on-limits backyard in an area that was not key locked. Children present were asleep at the time of the visit and Licensee stated that children had yet to go outside today. \302\267 Weapons are not present per Licensee. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations. \302\267 This is a one story home. \302\267 Verification of control of property on file. \302\267 Facility Sketch, Emergency Disaster Plan & Notification of Parent\342\200\231s Rights poster are posted. \302\267 Mandated Reporter Child Care Providers Training completed on 03/07/2025. \302\267 EMSA Pediatric CPR and First Aid Card completed on 03/16/2025. \302\267 Health & Safety Certificate \342\200\223 completed on 03/17/2020. \302\267 No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Clean, safe and age appropriate toys. During facility tour, LPA observed a play structure in the on-limits backyard which was in disrepair with portions which were cracked. Additionally, LPA observed several toys which had spider webs on them. Children present at the time were asleep and Licensee made items inaccessible during visit prior to children waking up and going outside. Furthermore, Licensee stated that children had not been outside today. LPA informed Licensee of Operation of a Family Child Care Home regulations and advised Licensee to check play areas prior to use by day-care children. \302\267 Upon LPA arrival, LPA observed several areas which are listed as off-limit areas did not have the gates up which made the areas off-limits. Day-care children present were asleep at the time and gates were put back up during visit. LPA reminded Licensee of regulation requirements. \302\267 Current roster on file. \302\267 Documentation of fire and disaster drills on file \342\200\223 Last drill conducted on 05/15/2025. \302\267 Children\342\200\231s records are complete. \302\267 Employee\342\200\231s records are not complete. During record review, LPA observed that S1 was missing the Mandated Reporter Child Care Providers Training (AB1207), proof of the measles immunization, and Tuberculosis (TB) Clearance that meets regulation requirements. \302\267 The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 Resident and/or staff records reviewed on 08/19/2025 indicate that all adults who require caregiver background checks have received all required clearances or exemptions. \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/ Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. During the exit interview, the Licensee Geno Randle, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Please note that Licensee Geno Randle was present upon LPA arrival and was present for a majority of the inspection. Licensee left the facility at 02:57PM and returned at approximately 03:10PM. Licensee left again at 03:19PM and returned at approximately 03:48PM. Exit interview conducted and report was reviewed with the Licensee Geno Randle.
