What the inspector wrote

Licensing Program Analyst (LPA), Samuel Lopez conducted a required/ an annual inspection as part of a compliance review. A tour of the inside and outside of the facility was granted and the following was observed and/or noted: This is facility consists of infants (24) and toddlers (12) from ages 6 months to 36 months. However, there were 8 infants and 21 toddlers. · The licensee/Site Supervisor is asked to update the following documents, if applicable, and submit to licensing within 30 days: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made) 4. LIC 309 Administrative Organization (only if changes have been made) 5. LIC 308 Designation of Administrative Responsibility (only if changes have been made) · The following items have been posted and are updated where necessary: - License - Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) - Parent’s Rights Poster (PUB393) - Personal Rights (LIC613A) - Child Car Seat Law - Menu · The facility is operating with the limits as stated on the license. · Ratios are being met during this inspection · Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards. · There are no weapons present at the facility as stated by Site Supervisor Imelda Contreras. · There is a self-draining splash pad located in the playground assigned for the toddler program. · Drinking water from a non-contaminating fixture or container is readily available both indoors and in the outdoor activity area. There are water fountains and pitcher filled with water and disposable cups available. · Medications are not being administered at this time · Hazards are stored where inaccessible to children which include Disinfectants, cleaning, sanitation solutions, and other items that are dangerous. · Poisons and toxins are locked. · All floors shall be clean and safe. · Highchairs and tables were observed safe and in good repair, during today’s inspection. · There are some changing tables within arm’s reach of a sink and others that are NOT; padding is at least 1” think with raised sides of at least 3” covered in washable vinyl or plastic. · Hand washing is completed before and after each diaper change and before feeding an infant. · Bathrooms were observed to be safe, sanitary and in operating condition; there is at least 1 potty chair for every 5 potty-training infants. · Playgrounds are enclosed by appropriate fences and free of hazards. · Outdoor activity areas are supplied with age and size appropriate equipment in good condition. · Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin. · Food is stored appropriately and protected from contamination. · All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. · Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request. · The areas around or under high climbing equipment, swings, slides, and similar equipment are/is cushioned with material that absorbs a fall by way of rubber matting. · Sign in/Sign out record was reviewed and meets regulation requirements. · A Staff member is present with current Pediatric CPR/First Aid which expires on 5/17/2027 · Opening and closing staff member’s CPR/First Aid expires on 5/17/2027 · Director completed Health and Safety Training - 5/18/2024 · A review of children’s records was conducted, and records were found to be complete during this inspection. · Disaster drills to be conducted every six months – last drill conducted on 2/19/2026 · The Licensee was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov · A review of staff records on 3/3/2026 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. · The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov · A review of staff records indicates that NOT all staff present meet minimum qualifications for the position for which they were hired. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Childcare Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. This facility was constructed after 2010 therefore is exempt from this requirement. - LPA discussed the safe sleep regulations with Site Supervisor Imelda Contreras and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. - LPA also informed Site Supervisor Imelda Contreras of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. - Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514- 0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/ . - Site Supervisor Imelda Contreras was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Site Supervisor Imelda Contreras was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California - The Duty Officer is available to answer questions Monday – Friday; 8:00am to 5:00pm at: 1-844-LET-US-NO (1-844-538-8766) and/or 951-782-4200 See LIC809-D for cited deficiencies. LPA Samuel Lopez informed Site Supervisor Imelda Contreras and Administrative Assistant Krystal Camargo that this report dated March 3, 2026 document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Samuel Lopez informed the Site Supervisor Imelda Contreras and Administrative Assistant Krystal Camargo to provide a copy of this licensing report dated March 3, 2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to Site Supervisor Imelda Contreras and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.     LIMITATIONS ON CAPACITY

    Based on observation and record review, the licensee did not comply with the section cited above. The facility was licensed and given a fire clearance for 12 toddlers and 24 infants however during today's inspection, LPA observed 8 infants and 21 toddlers. This poses an immediate health, safety or personal rights risk to persons in care.

  • CRIMINAL RECORD CLEARANCE

    Based on record review and observation, the licensee did not comply with the section cited above. Staff present at the facility had a criminal record clearance but not associated to the facility license. Although the licensee has a waiver for an administrative file for staff associations, the licensee has not requested to add this facility to that waiver. This poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on observation, the licensee did not comply with the section cited above. LPA observed infants in room A napping on cots/mats on the activity space near the napping room. Also observed that the infant/toddler area did not have a separate napping area, eventually the infants/toddlers were observed napping in the activity space. This poses/posed a potential health, safety or personal rights risk to persons in care.

  • INFANT CARE CENTER FIXTURES, FURNITURE, EQUIPMENT

    Based on observation, the licensee did not comply with the section cited above. LPA observed staff changing a child on a changing table that was not within arms reach of the sink. At the same time, there was another staff changing a diaper on the changing table that was near the sink. Both changing tables were located in the same space/area. This poses/posed a potential health, safety or personal rights risk to persons in care.

  • ARTICLE 2. Administration of Child Day Care Licensing

    Based on record review, the licensee did not comply with the section cited above. In reviewing staff records/files, LPA determined that staff #1 - #3, and staff #8 did not have or had not renewed their Mandated Reporter Training. This poses/posed a potential health, safety or personal rights risk to persons in care.
