What the inspector wrote

On the above noted date, Licensing Program Analyst's (LPA's), Diana Brasel and Griselda Castellon conducted an announced visit to add a toddler option component to their existing preschool single licensed component. Upon arrival, LPA's met with Celeste Etheridge Licensee and Michelle Gonzales Director. Licensee has requested to be licensed for 12 toddler option children and 48 preschool children. Upon completion of measuring the indoor room's today, the licensee provided an updated LIC 200A at time of visit to reflect the needed change in capacity due to indoor square footage measurements. The update capacity will be (8) toddler option children ages 18 months - 36 months in toddler room C and (48) preschool children ages 36 months through 5 in preschool rooms A & B. Days and Hours of operation will be Monday - Friday 6:00 am - 6:00 pm. Granted Fire clearance 12/05/2025. All indoor and outdoor activity space utilized for the children was inspected today. LPA's informed licensee that staff are required to maintain direct visual supervision of the children at all tis during indoor and outdoor activities. When medications are on site, director stated they are stored in a locked box in the kitchen. There are two small first aid kits that are not fully equipment located in the preschool classroom. The director stated they will purchase a fully equipped first aid kit. There is operational carbon monoxide detectors in all classrooms. All required licensing documents were observed posted in the entry area. Children will be signed in and out in the entry area. LPA's continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured for the toddler option children is 310. which is sufficient to accommodate the updated requested capacity of (8) children. Total indoor activity space measured for the preschool component is 1706.14, which is sufficient to accommodate the requested capacity of (48) children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, cots, napping mats, bookshelves, and other activity supplies for the children. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via children's brought water bottles, the facility has bottled water with paper cups if needed. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. LPA's observed a total of (1) sink and (2) toilet available, and (1) changing table for the toddler option children. See correction for an adult sink needed for the changing table. These are sufficient to accommodate the requested capacity of (8) toddler option children. LPA's observed (6) sinks and (4) toilets for the preschool children. These are sufficient to accommodate the updated requested capacity of (48) children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the office area. Facility provides breakfast, lunch and pm snack. The kitchen area currently includes an industrial freezer, industrial refrigerator, stove/oven, microwave, toaster, air fryer, and sink. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via a locked and latched dutch door. The facility currently has a two fully fenced playground areas. Fencing is block wall which is at least four feet high. The total square footage for the toddler option playground is 1322.73, which is sufficient for the number of children being requested. Shade is provided via the building and an umbrella. See correction for toddler option playground for sufficient outdoor age-appropriate toys and play equipment. The total square footage for the preschool playground is 17937.52, which is sufficient for the number of children being requested. There is a small climbing structure with a slide, which is age appropriate for preschool children and properly anchored. There is current rubber chips on the preschool playground for fall zones. Drinking water is available via children's water bottles or water with cups with bottled water provided by the facility. LPA observed all hazardous items on the playground to be inaccessible to children. Licensee were reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For the toddler option component there is a changing table has at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink will not used for meal preparation or dishwashing. Sleep logs are kept in room. Needs and Services Plans and Sleep Plans are kept in room. Licensee was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. This facility has an Incidental Medical Service Plan – IMS. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA reviewed and provided the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted. The facility has had a lead test completed within the last five years from today's visit. Visit date was (5/01/2024) and is using the same outlets. The following corrections are needed prior to the issuance of the updated license: Toddler Option: 1) Proof of an adult sink for the changing table shall be submitted. 2) Proof that the outdoor playground has had age appropriate toys and equipment installed along with cushioning material, shall be submitted. 3) Proof that some type of barrier has been installed to separate the area identified as off limits on the outdoor playground, shall be submitted. Preschool: 1. Proof of needed repairs to the mini merry go cycle has been completed. Repairs shall include all cracked seats to be replaced or repaired, all areas of rust shall be repaired and all areas of chipped paint shall be repaired. Licensee Celeste Etheridge understands that all proof of corrections must be provided to the Department within 30 days, or the request to add the toddler option component may be denied. Exit interview conducted and report was reviewed with the Celeste Etheridge Licensee.
