What the inspector wrote

On the date and time listed, Licensing Program Analysts (LPAs) Perla Ordonez and Samuel Lopez conducted a Case Management-Legal/Non-Compliance inspection for the purpose of informing the Licensee that this facility has been placed on probation to comply with the Stipulation And Waiver and Order that was ordered on 01/28/2021 for the licensee\342\200\231s previously licensed preschool program, facility number 334809747. Based on documentation that was provided to the Department by the licensee, the preschool program was tolled due to non-operation between March 16, 2020, and its closure on February 3, 2023. Per the Stipulation And Waiver and Order: \342\200\234The probationary period is tolled during any period when the facility is not operating\342\200\235. Thus, the probation term was not served for the stated 3 years. The probationary period shall be extended by the total time during which the facility is not operating. Please note that this Stipulation And Waiver and Order was originally ordered for the Licensee\342\200\231s preschool license which was previously listed under license number 334809747. A single license was issued on 6/18/2025 at the request of the licensee to reopen the Preschool Program. The Stipulation And Waiver and Order states that the Licensee, Renu Hope Foundation, shall comply with the following stipulations: - Submit an updated Plan of Operation that is applicable to all facilities operating under the corporate entity of RENU HOPE FOUNDATION and that clearly defines and describes the responsibilities of the position of a "Director" or "Executive Director" or similarly titled individual employed by Respondent RENU HOPE FOUNDATION who shall be responsible for the oversight and management of Respondent RENU HOPE FOUNDATION facilities. - Ensure that all individuals working, residing or volunteering in the facility shall obtain criminal record clearances, transfers, or exemptions prior to their initial presence in the facility and shall maintain proof of such criminal record clearances, transfers, or exemptions at the respective facility. - Maintain current personnel records of each employee and ensure that all employees have a current certificate of CPR and first aid training on file at the facility. - Ensure that poisons, detergents, cleaning compounds and other toxic products that could pose a danger to children are stored in a locked place where they are not accessible to children. - This Stipulation shall be posted in a conspicuous place at the facility for the duration of the probationary period. - Maintain an accurate, complete and current client roster at each facility which must be made available to the Department upon request. - Submit to the local licensing office a written summary of its hiring and training practices, including job descriptions for each position at each facility. - Report to the Licensing office the following: any unusual incident including, but not limited to, client death or injury which requires medical treatment, any suspected physical or psychological abuse of any client, any police, fire department, emergency personnel contacts at the facilities, and any physical plant changes and all unexplained absences. These incidents must be reported by the next working day, and a written report of the incident must be submitted within seven days following the occurrence of the incident. - Maintain current personnel records of each employee pursuant to Title 22 Regulation sections 101217. - Notify the local licensing office within one business day whenever substitute and/or temporary staff at a licensed facility location is 25% or more of the total number of personnel at the facility. - Ensure that medications and any other toxic products are properly stored and kept in a safe and locked place that is not accessible to persons other than employees responsible for the supervision of the centrally stored medication pursuant to Title 22 Regulation section 101226. - Ensure that each facility is clean, safe, sanitary and in good repair at all times. - Staff training shall include eight hours every three months. Respondent shall keep a record of staff who attend such training and make such records available to Licensing staff upon request. All such training must be provided using approved sources by the Department of Education or reputable outside organizations approved by the local Licensing office. - All newly hired personnel shall undergo 16 hours of training prior to placement in any facility, and thereafter 24 hours of training, once placed in the facility with a program vendor approved by the Department of Social Services local licensing office. - Complete Form LIC309 (Administrative Organization) annually and provide a copy of the completed form to the local Licensing office by no later than February 1 of each year of licensure. - Provide a copy of the corporate board minutes within 30 days of the date the board meeting to the local Licensing office. The effective date of the probation status is 06/18/2025. The probation status will cease on 06/18/2028. There were no deficiencies cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Jeanette Simms.
