Due to conflicting information obtained during interviews conducted from what was alleged, the evidence collected was not sufficient to substantiate or refute the above allegations. Although the allegations may have happened, or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. An exit interview was conducted, a copy of this report and Notice of Site Visit was provided to the facility representative, Leslie Torres. LPA observed the Notice of Site Visit was posted by staff. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS
