What the inspector wrote

On October 29, 2025, at 8:10 AM, Licensing Program Analyst (LPA) William Chancellor arrived at ABC CHILDCARE CENTER (CCC). LPA was greeted by Assistant Director (AD) Shannon Mouser and Mari Rivera. A case management visit is being conducted in response to the receipt of an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on October 11, 2025. DIR self reported an incident occurring on October 8, involving two daycare children being left unattended and found in a connecting classroom. The UIR was reviewed and confidential interviews were conducted with three staff (S1-S3) involved with the incident. Documents relevant to the incident were obtained and LPA confirmed that the facility was within ratio at the time of the incident. Interviews confirm that no children were left unsupervised in class 10 prior to combining with room 11. Based on information gathered, there is not sufficient evidence to corroborate that child 1(C1) and child 2 (C2) were left unsupervised for more then five minutes, due to using the restroom and eloping to the joining classroom. Staff 4 (S4) involved with incident, in no longer associated to the CCC. Based on information gathered, the facility acted appropriately, and there are no deficiencies cited at this time. An exit interview was conducted, and a copy of this report was provided to Assistant Director, Shannon Mouser and Mari Rivera. A Notice of Site Visit was provided and must remain posted for 30 consecutive days.
