What the inspector wrote

On December 11, 2025, at 3:01 PM, Licensing Program Analyst's (LPA's) William Chancellor and Courtnee Peebles conducted a Case Management visit separate from annual inspections. LPA's were greeted by Assistant Director (AD) Shannon Mouser and Mari Rivera. A case management visit is being conducted in response to the receipt of an Unusual Incident Report (UIR) received by Community Care Licensing (CCL) on November 14, 2025. DIR self reported an incident occurring on November 8, involving child 1 (C1) obtaining medical attention out of precaution due to pain coming from the wrist. The UIR was reviewed and confidential interviews were conducted with two staff (S1-S2) involved with the incident. Documents relevant to the incident were obtained and LPA confirmed that the facility was within ratio at the time of the incident. Relevant records confirmed C1 symptoms were linked to nursemaid elbow, not a fracture. Based on information gathered, the facility acted appropriately, and there are no deficiencies cited at this time. An exit interview was conducted, and a copy of this report was provided to Assistant Director, Shannon Mouser and Mari Rivera. A Notice of Site Visit was provided and must remain posted for 30 consecutive days.

    Based on observation, the licensee did not comply with the section cited above in two of eleven preschool classrooms. Where LPA observed more then ten sleep mats sheets to be in contact with other childrens bedding. Which poses a potential health, safety or personal rights risk to persons in care.
