What the inspector wrote

To meet operational needs, classrooms with low numbers of children or staff would routinely combine at nap time to allow staff to take their lunch breaks. Interviews and record review, denied that ratios are exceeded at nap, maintaining a ratio of one to twenty-four or less. Children are redirected to engage in quite activities on their nap mats, while other children remain asleep. A second allegation claimed that, childcare center does not remain sanitary and is not in good repair at all times. It was reported that classrooms 8 through 11 toilets, back up and overflow at least once a month. Six of six confidential interviews denied that toilets would overflow weekly, resulting in classroom carpet being soiled. During 2024 and 2025 annual inspections, LPA's confirmed each toilet in classrooms 8 through 11 to be operational. Records were not made available to corroborate any toilets requiring plumbing maintenance. The third allegation claims that staff alter incident reports. Six of six confidential interviews corroborate that management put eyes on the child involved and review the report before approving. LPA confirmed that ongoing training involves reports being routinely edited for grammar, spelling and necessary facts to protect confidentiality of both parties. Additionally, staff are trained to complete reports for incidents witnessed, regardless if the child involved is assigned to that staff\342\200\231s primary group. Six of six interviews denied that staff falsify reports, and staff are forced to sign. Due to conflicting statements and a lack of sufficient evidence, LPA was unable to corroborate the allegations that childcare center does not maintain proper teacher-child ratios, childcare center does not remain sanitary or in good repair at all times and staff alters incident reports. Although these incidents may have occurred, there is not a preponderance of evidence to prove or disprove the allegations. Therefore, the allegations are determined to be unsubstantiated. An exit interview was conducted, and a copy of this report, along with the appeal rights and a Notice of Site Visit, was provided to Assistant Director, Shannon Mouser. The Notice of Site Visit must remain posted in a prominent location, visible to families and caregivers, for 30 consecutive days.
