Inspector’s narrative

What the inspector wrote

disclosed that C1’s hand slipped as the child attempted to sit down at a table while staff was wiping it down; however, staff could not corroborate or provide details of the incident. Further interviews revealed that facility was within ratio and a teacher was in the area when the incident occurred, but no other staff witnessed it. A Record review of the child’s file disclosed that an ouch report was completed and provided with the necessary treatment/care to the parent. Regarding the allegation, staff left child in soiled diapers/clothing it was alleged that on March 7, 2023, and June 27, 2023, C1 was picked up from school and C1’s diaper had not been changed. It was also alleged that on June 27, 2023, C1’s shorts were soaking wet with urine and “poop”. Confidential interviews disclosed that on March 8, 2023, C1’s parent brought the incident to the Director’s attention, and the parent was shown the diaper logs that C1 had in fact been changed while at CCC. Confidential interviews also disclosed to LPA that the CCC staff was not aware of any other incidents or concerns regarding C1’s diaper not being changed while at CCC. Confidential interviews disclosed to LPA that the diaper logs are discarded at the end of each week and were not available to be reviewed by LPA. Confidential interviews also revealed that it is the CCC’s policy to check children’s diapers every hour; however, LPA was unable to corroborate if staff were adhering to that policy due to a lack of documentation and inconsistencies with the confidential interviews. Regarding the allegation that staff did not provide C1 with adequate food, it was alleged that on June 27, 2023, C1 had only eaten one apple slice and none of the snacks provided by parent had been opened. Confidential interviews disclosed that C1 was having a hard time adjusting and adapting to being at CCC and was only eating a few crackers or pretzels at the beginning but was coming around near the end of enrollment at CCC and eating more. Confidential interviews revealed that the CCC also provides meals and parents are welcome to provide food for their children as an alternative. Confidential interviews disclosed that C1 did not eat a lot initially at CCC, although it was alleged by the parent that C1 is a good eater at home and eats meals, snacks and bottles. In addition, confidential interviews disclosed that menus are provided to parents online and posted at facility for review. Also, the CCC has a policy to notify parents if there are any issues with a child eating while in care. Based on confidential interviews and records review, the allegations that due to lack of supervision, C1 was injured, staff left child in soiled diapers/clothing and staff did not provide child with adequate food, may have occurred, however are not supported or proven by evidence. Therefore, the allegations are unsubstantiated. A copy of this report, appeal rights and Notice of Site Visit were discussed and provided to Director. Director was reminded that the Notice of Site Visit must remain posted for 30 consecutive days.
