What the inspector wrote

Licensing Program Analysts (LPAs), Ana Noble, Lorena Valenzuela and Pauline Beschorner, Licensing Program Manager toured the inside and out of the facility. The facility is currently operating. Applicant is applying for change of ownership. The days and hours of operation will be: Monday through Friday, from 7 am to 6:30pm. The measurements were documented in Licensing Report LIC809 dated 9/14/2009 and 9/25/2017 for the current facility, license #334830693. Fire Clearance was granted on 5/31/2022. This is an initial application for Child Care Center (Preschool and Toddler Option) requesting 90 children. There is a request for a toddler option. Ages to be served are 3 years old to 6 years old (Toddler option 18 months to 36 months). In the application package, the applicant provided a disaster and mass casualty plan with all the required information. The property is owned St. Thomas Church and Pathways is operating the day-care and was previously licensed. LPAs and LPM arrived for the announced inspection at 8:51 am. Upon entry, there is the director's office around the corner there is a desk with the sign-in/sign-out sheets with an attached pen. Next to the Director\342\200\231s office, there is a bathroom with one sink and one toilet. The Director office is the isolation area. It has cots that are in good condition and age appropriate toys. The isolation area is located at the entrance of the facility (Director's Office). The facility was previous measurement both indoor space (Total of 4 Classrooms with one being the Toddler Option Class) and outdoor space and toilets/sinks counted. There is sufficient space for the request amount of 90 children. The staff restroom is located on the church side of the facility. In each classroom, there are age appropriate toys and furniture free of toxic substances. There is a carbon monoxide detector in hallway . There is a fully stocked first aid kit with ample supplies. There are no cleaning supplies in the classroom. Each classroom has garbage containers. There are water containers and cups in the classroom accessible to children to drink as they wish. The children nap on cots. The director confirmed there are no firearms on the premises. The kitchen is clean and free of rodents, flies, or other vermin. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm The following items were discussed with the Applicant/Director during inspection: \302\267 Menus shall be posted at least one week in advance in a place visible by the child\342\200\231s authorized representative, dated and kept on file for 30 days, and made available upon request. \302\267 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall \302\267 The Licensee was informed of their reporting requirements and provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov LPA discussed the safe sleep regulations with Applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant wa s reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Remove the weeds in the Toddler Option playground and submit a picture of correction. An exit interview was conducted, and this report was reviewed with the Tammi Reliford, Applicant . Appeal rights were discussed and provided during the exit interview.
