What the inspector wrote

On February 10, 2025, a complaint was received alleging the facility is operating out of ratio. LPA conducted interviews with seven witnesses. 4 of 7 witnesses stated the facility does not operate of ratio. LPA took census of each classroom and did not observe any classrooms out of ratio during the visit. LPA conducted a record review and found the sign in/sign out sheets did not show any conflicting evidence. Based on LPA’s observations, record review and interviews conducted, the allegation of the facility operating out of ratio is unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Appeal rights were issued and discussed with licensee and their signature on this form acknowledges receipt of these rights. Exit interview was conducted and report was reviewed by director, Suriati Cafaro and assistant director, Jacquelyn Negrete. A notice of site visit was given to director and must remain posted on, or immediately adjacent to the interior side of the main door for 30 days. The report must be made available to the public for three years. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
