Regarding the allegation that staff did not follow proper reporting requirements, interviews confirmed that the facility did not report the communicable disease outbreak to the licensing department, the local health department, or to parents. Based on interviews, observations, and evidence collected, the Department has determined that the allegations, that staff did not prevent the spread of HFMD and did not follow proper reporting requirements, are substantiated. An exit interview was conducted, appeal rights were explained, and a copy of the report was provided to Licensee Nam De Silva.
