What the inspector wrote

from disputing over the same items. Following a comprehensive investigation, including interviews and a review of records, LPA Pulido was unable to corroborate the allegation that staff do not prevent children from being injured by another child. The Department determined that the allegation is unsubstantiated. A finding of unsubstantiated means that although the allegation may have occurred or may be credible, there is insufficient evidence to prove or disprove the alleged violation. An exit interview was conducted with Director Julia McCarthy, during which a copy of the report and Appeal Rights were provided. A Notice of Site Visit was issued and must remain posted at the facility for 30 days.
