What the inspector wrote

to gain their attention. Staff reported they had not witnessed the Licensee yell at children or handle them aggressively; however, some interviews indicated that the Licensee had been observed being “snappy” or raising her voice with children. No interview provided evidence of physical or inappropriate handling. Regarding the allegation that the Licensee failed to ensure a safe environment, the concern involved the front door allegedly being left unlocked and children running into the street. Interviews indicated that children are not allowed near the front door unless a staff member is present. Baby gates are placed near the front door to prevent children from exiting, although parents and staff open the gates when entering the home. During the initial inspection, LPA observed the baby gate left open. Parents interviewed stated they had not seen or heard of children exiting the home unsupervised. The final allegation involved the facility operating out of ratio, specifically that one staff member would supervise fourteen children after nap time. Interviews revealed that the Licensee typically has four staff members present during operating hours to ensure compliance with ratio and capacity requirements. Additionally, during the November 5, 2025 visit, LPA observed the Licensee and one additional staff member supervising ten children, which was within the required ratio. No evidence was obtained to support or deny the claim that the facility operated out of ratio at any time during the investigation. Based on the information obtained throughout the investigation, there is insufficient evidence to substantiate the allegations that the provider yelled at day care children, handled children in an inappropriate manner, failed to ensure a safe environment, or operated out of ratio. Although the allegations may have occurred or may be valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. Therefore, the allegations are deemed unsubstantiated at this time. An exit interview was conducted, and a copy of this report, the appeal rights, and the Notice of Site Visit were provided to Licensee Destiny Frye. The Licensee was reminded that the Notice of Site Visit must be posted in a visible area of the facility for 30 days.
