What Happened

Subsequent case management visit following child elopement incident on 01/08/2025 when child left facility unsupervised through front door and was found at nearby park. Investigation found violation of supervision regulations. Facility cited with Type A citation and $500 immediate civil penalty.

Observations

  • Child left facility unattended through front door while licensee in restroom for approximately 2 minutes
  • Child found at Shady Maple Park 0.2 miles away by community member who called police
  • Child was unsupervised for approximately 6-10 minutes
  • Violation: children not supervised at all times per CA Code of Regulations Title 22 Section 102417(a)
