What the inspector wrote

On 03/26/25, at 1:05 pm, Licensing Program Analyst (LPA) Naomi Hurtado arrived the facility to conduct a case management visit. A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was dated 01/16/2025. LPA met with License Rosa Ceja advised her of the reason for the visit. During the visit LPA reviewed documents and conducted an interview with the Licensee. Staff and child involved in the UIR were not present to interview. LPA Hurtado reported to the Licensee that she was not in compliance with supervision regulations despite her appropriate response to the incident. Licensee will receive a Type B deficiency at this time. See attached 809-D page. A notice of site visit was given to licensee and must remain posted and in view of the public for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted with Licensee Rosa Ceja. Licensee was provided with a copy of this report and appeal rights were discussed.
