What the inspector wrote

On the above date and time, Licensing Program Analyst (LPA) Sumayya Habeebulla arrived at the Early Learning Academy (CCC) to conduct a case management visit regarding an Unusual Incident Report (UIR) received by CCLD on August 29, 2025, for an incident that occurred at the CCC on August 22, 2025. During the interview, it was stated by Staff #1 (S1) that on 08/22/2025, it was Child #1’s (C1) first day of school. C1 arrived and was crying during the drop off. The classroom aide sat with C1 in the library area and helped them calm down. C1 was observed to calm down. As the class was getting ready to wash their hands for lunch, the classroom aide assisted C1 to join the line. While C1 was walking towards the sink to wash their hands, the child abruptly stopped crying, and it was observed by the staff that C1’s eyes moved to one side and the child’s body jerked for a few seconds. C1’s eyes returned to normal position and the child began to cry again. Staff immediately contacted the front office and the school nurse. Facility administration contacted parents who also arrived shortly. In the interim the child sat at the lunch table with peers and was offered lunch. Child did not have lunch and laid their head down on the table to rest. C1 was picked up by parents shortly. As per the facility follow-up, C1 did not require any medical attention. Staff in all classrooms have an Centegix emergency quick response button to alert the front office and administration of any situation that needs assistance. LPA Sumayya Habeebulla determined that the facility took the necessary steps to ensure the safety of the children. Based on the information obtained during the visit, there appears to be no violations of Title 22 Regulations pertaining to the reported incident. An exit interview was conducted, and a copy of this report was provided. A copy of this report must be made available to the public, at the facility site, for 3 years.
