Inspector’s narrative

What the inspector wrote

On the above noted date and time, Licensing Program Analyst (LPA), Diana Brasel conducted an announced follow-up Pre-Licensing inspection for the request to add an infant program component. Currently the facility has an existing pre-school program license. Upon arrival, LPA met with Lucy Coronado, Director. Applicant is requesting to be licensed for ( 4 ) infants, ages 6 weeks - 24 months in the Infant Room. Hours of operation will be Monday - Friday 7:30 am - 5:30 pm for the infant program and Monday - Friday 6:30 am - 6:00 pm for the pre-school program. On todays visit the licensed preschool component had 28 children to 6 staff. Days and hours of operation for the preschool component is Monday - Friday 6:30 am - 6:00 pm. The following corrections were needed from the 05/24/24 initial pre-licensing: 1. Updated Mandated Reporter Child Care Provider training for Director, Lucy Coronado. 2. Once the infant playground has been enclosed with an at least 4 foot tall fence, an updated facility outdoor sketch to show the infant and preschool playground will be needed. 3. An updated facility indoor sketch to show the added activity space and gated areas. On today's visit LPA obtained a current Mandated Reporter Child Care Provider training for the Director, Lucy Coronado, an updated indoor infant facility sketch, and an updated outdoor infant playground sketch. In addition LPA obtained an updated sketch for the preschool program. The facility has taken a portion of the current designated outdoor activity space from the preschool playground which was being used by the younger preschool children and has converted it into the infant playground. The total square footage for all the outdoor activity space for the current licensed preschool program is now 13,135.00, which is sufficient for the current licensed preschool component capacity of 82. On todays visit LPA measured the indoor space and outdoor activity space for the requested infant component. LPA informed Lucy Coronado that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. Total indoor activity space measured 192.33 which is sufficient to accommodate the requested capacity of 4 children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment. Located in the napping area are 4 cribs and cots are available for infants who can climb out of a crib. Toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Fire clearance was granted on 2/21/2024. There is at least one toilet for every 4 infants and 1 sink available for children's use, which is sufficient to accommodate the requested capacity of 4 children. The facility currently has a fully fenced playground area which is separate from the other component. Fencing is a block wall, wrought iron, and chain link fencing. The fencing that separates the infant and preschool outdoor playground activity space, currently does not meet the four feet high requirement. The total square footage for all the outdoor activity space is 1,280.00, which is more than sufficient to accommodate the requested capacity. Shade is provided via wind sail, canopy awning off of building, and trees. On one half of the infant playground the facility has installed rubber cushioning material. Per the Director the staff will be using the rubber padded cushioning area for outdoor activity play space and the concrete area outside of the infant room will be used for tricycles or other similar items. The director has also stated, the facility will be adding additional cushioning material to the concrete section of the infant playground, currently there is sufficient space for the requested capacity on the rubber padded area of the outdoor activity space. There are sufficient outdoor age-appropriate toys and play equipment available on the padded playground area. The following correction is needed prior to the issuance of the license: 1. A picture to show that the fence separating the preschool and infant outdoor activity space is at least four feet tall. Director Lucy Coronado understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Upon LPA receiving a picture showing the fencing between the outdoor preschool and infant activity space is four feet tall, the application will be submitted for approval of the request to add the an infant component. A notice of site visit was given to Director and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. \342\200\234 Exit interview conducted and report was reviewed with the Director Lucy Coronado.
