What the inspector wrote

On 05/16/25, Licensing Program Analyst (LPA), Kelli Waters, conducted an unannounced Case Management visit to follow up on Unusual Incident Report (UIR) that was submitted to Licensing by the facility on 04/18/25, regarding an incident that took place on 04/14/25. LPA met with Director, Roxanne Walker to discuss incident. The Director reported the following: on April 14, 2025, Staff #1 (S1) reported that while outside, Child #1 (C1) fell of the slide and landed on left arm. Parent of C1 then reported later that evening that C1 was taken for emergency medical care and C1 had suffered a fractured elbow. LPA Waters conducted a visual inspection of the playground, reviewed documents, and interviewed staff. After investigating, LPA Waters determined that the facility took the necessary steps to ensure the safety of the children and visual supervision was maintained during outside play. Based on the information obtained during the visit, there appears to be no violations of Title 22 Regulations pertaining to the reported incident. LPA Waters reminded Director that according to regulation 101212(d) unusual incidents must be reported by telephone or fax by the next working day and that a written report must be submitted within 7 working days. An exit interview was conducted, and a copy of this report and appeal rights were provided. A copy of this report must be made available to the public, at the facility site, for 3 years.
