What the inspector wrote

On the above noted date and time, Licensing Program Analyst Diana Brasel arrived at the facility to conduct a follow-up Pre-Licensing inspection for a new infant component, currently the facility has a preschool license, which capacity will decrease. This facility will become a single licensed facility for preschool and infant. Upon arrival, LPA met with applicant/licensee America Figueroa. Applicant is requesting to be licensed for (12) infants 0 - 24 months of age in the infant room and 7 preschool children ages 2 through 5 years of age in the preschool room. Hours of operation will be Monday - Friday 6:30 - 5:30 pm. No children were present on today's visit. An initial pre-licensing visit was conducted on 12/02/2024, at which time additional space was needed for the infant playground and adjustments were needed for the infant napping area. A return visit was needed to measure the infant playground and the infant room once corrections were completed. On todays visit LPA measured all indoor and outdoor activity space for the infant component. Total indoor activity space measured for infant is 437.00, which is sufficient to accommodate the requested capacity of 12 children. No changes were made to the Indoor space for preschool which was measured on 12/02/2024 and is 260.06, which is sufficient to accommodate the requested capacity of 7 children. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, cribs, napping mats for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. The napping area has 6 cribs with a solid napping wall. LPA discussed with the licensee that a qualified staff shall be present at all times in the napping area when a child is sleeping. Fire clearance was granted on 07/24/24. High chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. High chairs are made of washable, moisture-resistant material. The changing table has at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via a solid wood wall. This barrier is at least four feet high, made of sound absorbing material. Sleep logs will be kept in the infant room per the licensee. Needs and Services Plans and Sleep Plans will be kept in the infant room. There is one children's sink, one potty chair, and a changing table available for the infants. These are sufficient to accommodate the requested capacity of 12 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the office/reception area. Facility will provide breakfast, lunch and pm snack. Meals are delivered by catering service. The kitchen area currently includes a refrigerator, microwave, and sink. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via a locked door. The facility currently has a fully fenced playground area which is separate from other components. fencing chain link, wooden fencing, and block wall. The total square footage for all the infant outdoor activity space is 363.93, which is not sufficient to accommodate the requested capacity of 12 children. LPA's estimated the approximate area on 12/02/2024, LPA Brasel measured the newly enclosed area on today's visit. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade is provided via an easy up canopy. There are sufficient outdoor age-appropriate toys and play equipment available. The below needed corrections were needed from the original pre-licensing visit conducted on 12/02/2024, all corrections were verified and cleared on this date, 12/20/2024. 1. Proof of an approved changing table installed shall be submitted- (Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing) 2. Proof that the front bay window located in the infant room that has metal with sharp areas and splintered chipped wood has be repaired or made inaccessible. 3. Proof that all holes on the walls in the infant room have been repaired shall be submitted. 4. Proof that shade has been added to the infant playground and that the playground has been extended as discussed at time of pre-licensing visit. A return visit will be required to re-measure once proof of correction has been received. 5. The applicant shall submit a waiver and provide a schedule to share the infant playground. The requested waiver will be processed for approval, once approved the approval for the added infant component will be processed. Exit interview conducted and report was reviewed with the applicant/licensee America Figueroa. A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
