What the inspector wrote

Licensing Program Analyst (LPA) Tricia Danielson conducted this case management visit to address deficiencies discovered during the investigations for complaint control #10-CC-20250806084209 and #10-CC-20250904095231. On 9/3/2025 while conducting a visit at the facility, LPA observed several infants napping in cribs with prohibited items such as blankets, pillows, and toys. During the visit, LPA provided consultation to Licensee Figueroa regarding safe sleep requirements. During an investigative review of records, LPA discovered the Infant Needs and Services Plan for Child #3 (C3) had not been updated since May 15, 2025. Per regulation, all Infant Needs and Services Plans must be updated at least quarterly. Additionally, the facility's own Infant Needs and Services Plan form indicated the plan will be updated every three months until the child is two years old. LPA also discovered the current Admissions Agreement the facility is utilizing defers from the approved Admissions Agreement on file. Community Care Licensing (CCL) was not aware of any changes made to the facility's Admission Agreement. Therefore, California Code of Regulations (Title 22) deficiencies are being cited on the attached 809-D page. An exit interview was conducted, and this report was reviewed with and provided to Licensee Figueroa. Appeal Rights were also discussed and provided. An LIC 9213- Notice of Site Visit was also issued and must remain posted near the main entrance for 30 days. Non-compliance with posting will result in a $100 fine. This report must be accessible to the public for three years.

  • MODIFICATIONS TO INFANT NEEDS AND SERVICES PLAN

    Modifications to Infant Needs and Services Plan- (a) The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. This requirement was not met as evidenced by: Based on record review, the Licensee did not comply with the section cited above. C3's Infant Needs and Services Plan has not been updated since 5/15/25. This poses a potential health, safety, and personal rights risks to children in care.

  • INFANT CARE CENTER SLEEPING EQUIPMENT

    Infant Care Center Sleeping Equipment- (f) Cribs shall be free from all loose articles and objects, including blankets and pillows. This requirement was not met as evidenced by: Based on observation, the Licensee did not comply with the secion cited above. LPA observed loose articles and objects in several cribs with napping infants. This posed a potential health, safety, and personal rights risk to children in care.
