What the inspector wrote
On 04/17/2025 at 8:15 am, Licensing Program Analyst (LPA) Gabriela Hernandez conducted an announced pre-licensing inspection for a large family childcare at the above home. LPA met with Applicant Ivonne Cortes, who guided LPA on a tour of the home. Per Applicant, hours of operation will be Monday through Friday from 7:00AM to 5:00 PM. Applicant states they will provide care for children 6 months to 10 years of age. The fire clearance for a large family child care was approved on 04/01/2025 by Lake Elsinore Fire Department.
This is a two-story home consisting of 4 bedrooms 2 1/2 bathrooms, a living room, dining room, daycare room, kitchen, laundry room, front yard, backyard and attached garage. The home was inspected for safety, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children are kept in the Garage which has 2 locks and a level handle lock in place.
Areas used by children include: Day Care Room, Living Room, Dining room, and ½ bathroom downstairs. Areas off limits include: Entire 2
nd
floor (All bedrooms and Laundry Room are located on 2
nd
floor), Kitchen, Backyard, and Garage. The stairs are barricaded preventing access to the 2
nd
level. There is a safety gate installed preventing access into the kitchen. Knives and other sharp utensils are in an upper kitchen cabinets, the kitchen is off-limits. Medicines are stored in an upper kitchen cabinet as well, the cabinet has a safety lock in place as well.
The home is clean, neat, and orderly, there are outlet covers throughout the home. LPA observed age-appropriate toys and learning materials.
Smoke/carbon monoxide detector was observed, tested by applicant, and found to be operable. The required (2A-10BC) fire extinguisher was observed behind the front door. The 1

aid kit is located in the day care room. LPA reminded applicant that the fire extinguisher must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors should be checked, batteries should be replaced.
Applicants have completed the required preventative health and safety course which includes nutrition and prevention of lead exposure on 02/29/2020 and 10/18/2021. Applicant’s Pediatric First Aid and CPR expires 10/26/2026. Mandated Reporter Training expires 12/09/2026. Applicants have required proof of immunizations on file..
Per Applicant, there are not any weapons or firearms in the facility. Applicant understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations.
According to the applicant, the backyard will be off-limits until it is ready for day care children. Applicant understands they must contact licensing, submit updated facility sketch and obtain approval prior to making it a part of their day care.
There
are not
any pools, spas, or other bodies of water. Licensee understands that all bodies of water including ponds, above ground pools, spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar products must be emptied immediately after use and stored in an upright position.
LPA reviewed with applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.
The applicant provided proof of control of property. Because the applicant, rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC9149).
Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
LPA informed Applicant(s) of their reporting requirements. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. Fire and safety drills must be performed every six months and documented for review by the Department. Smoking is not allowed in a home that is licensed as a family day care home, and in those areas of the family day care home where children are present.
A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility.
Annual fees must be paid promptly and by the due date or a late fee shall be assessed, and/or the License shall be terminated.
Changes should be reported to the Department as soon as they occur such as construction and remodeling, telephone number changes and/or if you move from the home.
Applicant states they will provide food for the children. Applicant states they will not provide transportation.
The following items were also reviewed with the applicant during inspection:
- Failure to meet the posting requirements shall result in an immediate $100 civil penalty.
- Responsibilities of being a mandated reporter and updating training every 2 years
- Personal rights of children in care, including no corporal punishment
- Responsibility to know the Title 22 Regulations for anyone providing care and supervision
- Capacity and Supervision requirements
- Inaccessibility of hazards must be constantly reassessed depending on the children in care
- Baby walkers, bouncy seats, and other similar items are prohibited
- Once licensed, the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809/LIC9099) must also be posted for 30 days. A civil penalty of $100 per violation will be assessed for noncompliance.

Applicant(s) was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
On this date, 03/10/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

The application for a Large Family Child Care Home will be submitted for approval with a maximum capacity of 12 or 14 with parent notification.
An exit interview was conducted, and a copy of this report was reviewed with and handed to Applicant Ivonne Cortes.
