On October 23rd 2025, Licensing Program Analyst (LPA) Mandie Goodwin and Licensing Program Manager (LPM) Seychelle De Luca met with Samantha Subuyuj to conduct an unannounced case management inspection. During todays inspection there were 57 preschool children supervised by 19 staff across six classrooms. The purpose of today's inspection was to create a plan of correction following reports of lead exceedances in two of the facility outlets. On August 21st 2025, the facility tested their water samples for lead. Two water outlets sampled indicated an Action Level Exceedance (ALE). These outlets are marked (R) and (I) on the lead sample results and indicates a faucet and hand held attachment in the kitchen. Water tested at 5.6 parts per billion (ppb) in outlet I and 6.6 ppb in outlet R, which exceeds the threshold value of 5.5ppb. Signs have been placed on the outlet stating that the outlets should not be used for drinking or food preparation. Director stated that the sink is used for rinsing dishes prior to going through the dish washer, and was previously used for rinsing produce. Director stated that they have a second sink that they will now use for food preparation that tested below the lead exceedance level. Facility is flushing and retesting the outlet. LPA received the lead testing map and other required documents (LIC 9275 External Water Sampler Self-Certification Form and LIC 9276 Child Care Center Sampling Checklist Form). A deficiency is cited on the following LIC809-D and the plan of correction was reviewed with Samantha Subuyuj. A copy of this report and appeal rights were provided. Notice of Site visit was provided and should remain posted for 30 days.
