What the inspector wrote

On February 7, 2023 at 9:00 AM, Licensing Program Analyst (LPA) Tanya Washington met with Facility Representative, Harpeet Heer for an unannounced case management inspection regarding two separate self- reported unusual incidents. Upon arrival, LPA observed 15 preschool age children in the preschool room #3 supervised by five staff and 5 preschool age children in classroom #1 supervised by three staff. Shortly after, Program Manager Karen Hamman and Centers Manager, Tarra Kleiser arrived to assist with the remainder of the inspection. During today\342\200\231s inspection LPA toured the facility grounds and conducted interviews with staff regarding the occurred incidents. The facility self- reported that on 12/12/2022 during the transition of children from playground to classroom, Child #1 was left outside. Staff interviews indicate that a roll call was conducted and Child #1 was accounted for, at some point Child #1 ran off as the children transitioned inside. Staff stated approximately 15 seconds after the door was closed, the door bell rang and they observed Child #1 entering the classroom. The facility self- reported that on 01/13/2023, three staff members were supervising a group of children outside and it started to rain. Staff #2 and Staff #3 started lining up the children under the overhang of the building to prevent them from getting wet. Staff #1, stayed outside on the playground to put away the toys. After Staff #1 was finished putting away the toys, they went inside the classroom. After the group of children entered the classroom, Staff #2 started checking off the children using the roll sheet, realizing Child #2 was not inside. Staff #1, went back out to the playground and retrieved Child #2 who was at the top of the slide unharmed. Interviews with staff indicate that Child #2 was without supervision for approximately one minute. Since the incidents, upper management conducted a meeting with all staff regarding prevention of incidents, supervision practices and supervision guidelines were discussed. Facility Representative will monitor transitions to ensure roll call and counts are being completed consistently and accurately. Beanstalk Management will continue to do unannounced visits to the site to monitor supervision practices. LPA Tanya Washington informed facility representative Karen Hamman that this report dated 02/07/2023 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Tanya Washington informed the facility representative Karen Hamman to provide a copy of this licensing report dated 02/76/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility representative Karen Hamman. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
