What the inspector wrote

On October 10, 2024, at 12:40 PM, Licensing Program Analyst (LPA) Tanya Washington met with Site Supervisor Harpeet Heer for an unannounced case management inspection regarding a self reported incident which occurred on 10/02/2024. During the inspection, Center Manager Tarra Kleiser and Assistant Centers Manager Tiffany McPherson arrived to the facility to assist LPA. Upon arrival LPA observed care and supervision of 16 children supervised by four staff. It was reported that on 10/02/2024 approximately at 1:30 PM, Child #1 exited classroom #3 without staffs knowledge. LPA learned that Child #1 was being assisted in the restroom by Staff #1. When both exited the restroom Staff #1 returned to the restroom to clean the area. There were two other staff in the class tending to other children who did not observe Child #1 exit the classroom. Child #1 returned to the classroom approximately 27 seconds later. Staff #2 was near the classroom door and observed Child #1 re-enter the classroom and reported the incident to management. LPA learned from management that the incident was recorded on surveillance camera which was reviewed by LPA. LPA also learned that the facility has notified the parent of the incident. Since the incident, staff has repositioned the cubbies that were blocking the entrance door from view. The facility staff has also changed the policy to not turn off the door chime during nap time to alert them who enters and exits the classroom. A copy of this report along with the following page LIC809D shall be provided to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A civil penalty in the amount of $500.00 is assessed on the following page for absence of supervision. Exit interview conducted and report was reviewed with Facility Representative Tarra Kleiser . A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights are provided.

Child exited the classroom into a fenced outdoor play area and reentered the classroom approximately 27 seconds later. This is an immediate risk to the health and safety of children in care. A civil penalty in the amount of $500.00 is assessed.
