What the inspector wrote

On Tuesday August 5 th 2025, Licensing Program Analysts (LPAs) Mandie Goodwin and Fabian Schwartz met with Program Coordinator Paula Cook for the purpose of an unannounced random inspection. LPA toured the facility at which time a census of 86 school age children supervised by 7 staff members were observed. The facility hours of operation are Monday through Friday 7:00 AM to 6:00 PM. Facility operates in portable rooms 1, 2, and 3. A health and safety inspection was conducted in the classrooms, restrooms, food service area, and outdoor play areas. Facility provides snacks and lunches are brought from home. LPA observed all postings. Cleaning disinfectants and hazardous items are appropriately stored and inaccessible to children. Furniture and equipment are in good condition, and toileting facilities are in sanitary and operating condition. There are bins for solid waste. The floors appeared clean throughout the facility. LPAs observed smoke and carbon monoxide detectors, and a fire extinguisher. LPA observed a current roster, documentation of sign in and sign outs conducted via KidsHub, and documentation that facility is conducting emergency disaster drills at least every 6 months. Per record the last fire drill was conducted in May. Facility follows the school fire drill schedule. Children have access to water indoors and outdoors by bringing water bottles which are refilled. Playground equipment and surfaces were inspected and are in good condition, with enough resilient material under climbing structures to absorb a fall. There are no firearms or bodies of water on the premises. Director stated there are no poisons on the premises. Continued on 809-C LPA reviewed present staff files. At least one staff member present today has current Pediatric CPR and First Aid certification. LPA observed immunization records and documentation of the educational background, training, and/or experience of staff. Upon review, all staff members had current mandated reporter trainings. LPA advised AB 1207 Mandated Reporter training must be renewed every two years and can be renewed at www.mandatedreporterca.com . A sample of 6 children's records were reviewed. Each child's file contained an emergency card, consent for emergency medical treatment, health history, a record of immunization\342\200\231s, and notifications of personal and parent\342\200\231s rights. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Continued on 809-C LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information- https://www.cdss.ca.gov./inforesources/child-care-licesing/water-testing-information LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Paula Cook. Based on the areas evaluated, a Type B title 22 deficiency was cited today. Appeal rights were provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
