Inspector’s narrative

What the inspector wrote

At 10:00 a.m. on Friday April 29th, 2022, Licensing Program Analysts (LPAs) Karyn Guerra and Amanda Sutter met with Director, Meghan Mara for the purpose of an unannounced complaint inspection. It was alleged that the daycare toilet is in disrepair. Throughout the course of the investigation, LPAs conducted an interview and made observations. LPAs observed two toilets in the boy's restroom. LPA was unable to flush the toilet on the left side. Toilet on the right side was observed to have a slow flush and water fill. There is a preponderance of evidence to support the above allegation; therefore, the finding is SUBSTANTIATED. Title 22 deficiencies are cited on the subsequent pages of this report. Appeal rights were provided. Director's signature on this report acknowledges receipt of these rights. This report was reviewed with the Director. A notice of site visit was provided and shall be posted for a period of 30 days for parental review.
