What the inspector wrote

On April 9 th 2026, Licensing Program Analyst (LPA) Mandie Goodwin met with Program Developer (PD) Cynthia Clemo for the purpose of an unannounced random inspection. LPA toured the facility at which time no children were present for the 4 th R program. The facility hours of operation are Monday through Friday from 7:00 AM to 6:00 PM. A health and safety inspection was conducted in the portable classrooms, school restrooms, food service area, and outdoor play area. LPA observed all postings. Cleaning disinfectants and hazardous items are appropriately stored and inaccessible to children. Furniture and equipment are in good condition, and toileting facilities are in sanitary and operating condition. There are bins for solid waste. The floors appeared clean throughout the facility. LPA observed smoke and carbon monoxide detectors, and a fire extinguisher. LPA observed a current roster, documentation of sign in and sign outs conducted on KidsHub, and documentation that facility is conducting emergency disaster drills at least every 6 months. Per record the last fire drill was conducted on March 11 th , 2026. Children have access to water indoors and outdoors by bringing water bottles which are refilled using filter water fountains. Playground equipment and surfaces were inspected and are in good condition, with enough resilient material under climbing structures to absorb a fall. There are no firearms or bodies of water on the premises. PD stated there are no poisons on the premises. LPA reviewed a sample of three staff files. At least one staff member present today has current Pediatric CPR and First Aid certification. (Report continues 809-C) LPA observed immunization records and documentation of the educational background, training, and/or experience of staff. Upon review, all staff members had current mandated reporter trainings. LPA advised AB 1207 Mandated Reporter training must be renewed every two years and can be renewed at www.mandatedreporterca.com . A sample of five children's records were reviewed. Each child's file contained an emergency card, consent for emergency medical treatment, and health history. Facility additionally has digital files on kidshub which contains the notifications of personal and parent’s rights. PD was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) CCC completed testing and no lead exceedances (Report continues 809-C) Incidental Medical Services (IMS) policy was discussed.
