What Happened
Case management inspection initiated following lead exceedance findings. Facility retested water samples on 02/21/2025 and identified one outlet exceeding Action Level for lead. Plan of correction in progress with signage and restricted use implemented.
Observations
- Action Level Exceedance (ALE) detected: 6.880 parts per billion (ppb) in outlet A (sink in room 1), exceeds safe level of 5.5 ppb
- Faucet D was retested and renamed as faucet A on 02/21/2025
- Director states faucet has not been used since testing
- All other previously tested outlets have been cleared
