What the inspector wrote

On 06/25/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted a Licensee Initiated Case Management inspection for the purpose of a room change. LPA arrived at the facility and was met by Director Antoinette Leon-Martinez (D1). LPA disclosed the purpose of the inspection and was granted entrance. The current hours of operation are Monday thru Friday 7:00 am until 6:00 pm INDOOR ACTIVITY SPACE: The program has requested that the preschool license (#343614216) temporarily be inactive (07/01/2025 to 06/30/2026) and that the school age program utilize the current preschool classroom and bathrooms. A Fire Clearance, dated 03/28/2007 , is on file with the Department. LPA observed a total of 2 sinks in the classrooms. LPA inspected the bathrooms and observed 2 toilets and 2 additional sinks. LPA observed the facility has a total of 2 toilets and 4 sinks. This is sufficient to meet the needs of 56 school aged children which the facility has a capacity for. Licensee is not making a change to the licensed capacity. LPA used the current measurements on file for classrooms #1, report dated 02/15/2007. The classroom #1 contains a total measurement of 769.16 square feet which is in addition to the current classroom #2. Both classrooms will accommodate the licensed capacity of 56 school aged children. LPA will approve the use of classrooms 1 and the bathrooms for school age children, once the Application for Room Change is received and after 07/01/2025, when the preschool license in inactive. No deficiencies were cited today in the areas that were evaluated. An exit interview was conducted, and the report was reviewed with Director Leon-Martinez. Licensee Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100.
