Inspector’s narrative

What the inspector wrote

At 9:00am on 2/20/2026, Licensing Program Analysts (LPAs) Matthew Gallo and Joshua Hatch met with facility representative Robin Rivas to conduct a case management visit related to deficiencies observed in the course of a complaint investigation. Upon arrival, LPA observed a total census of 11 school aged children supervised by 1 staff. Based on observations during both the investigation and today's visit, LPAs determined that the facility maintains one toilet and one sink for the 45 total capacity on the license. The director stated that they are not aware of the facility ever providing care to 45 school aged children at one time, and that the current daily census of children is between 15-20 depending on the day. Per licensing regulations, one toilet and would sink should be available for every 15 children. A Type B Title 22 deficiency is cited on the following LIC809-D Exit interview conducted and report was reviewed with the facility representative, Robin Rivas. A notice of site visit was given and must remain posted for 30 days. LPAs provided appeal rights.
