Licensing Program Analyst (LPA) Loraine Perez met with Facility Representative, Krystle Mitchell, for the purpose of conducting an unannounced subsequent complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained to Facility Representative. During today's inspection, LPA conducted interviews, and observed care. Witness statements, LPA observations, and document reviews failed to corroborate the allegation. The facility notified families of pest control application and temporary room changes. Furniture and storage boxes were moved to the back of the classroom and kept out of reach of the children with supervision and barricade. Although the allegation may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Krystle Mitchell. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
