What the inspector wrote

A deficiency was cited during today's inspection. This report was reviewed with Director, and an exit interview was conducted. Appeal Rights and A Notice of Site Visit (NOS) was provided to Director, who will post it for a period of 30 days for parental review. LPA Interviews and observation failed to corroborate the allegation that staff does not provide a clean and sanitary environment for daycare children, and staff did not follow safe food practices. Although the alleged violations may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that it did or did not occur, therefore, it is UNSUBSTANTIATED.

  • TEACHER-CHILD RATIO

    101216.3 Teacher-Child Ratio(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below...This requirement was not met as evidenced by: It was determined by LPA interviews that the facility is operating out of ratio in more than one occasion. Interviews revealed that there has been occasion in the past when some other classrooms were operating out of ratio when the facility was short staffed. Staff interviews revealed that they accept a child in preschool, placing them out of ratio, and then call for assistance. Staff stated that help will come in, but staff will be out of ratio for about 5 minutes.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    101229 Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time...shall include visual observation.This requirement is not met as evidenced by: Based on interview, and record reviews, Director (D) did not comply with the section cited above as a child was left in the playground without supervision of staff for approximately 1 minute, which poses an immediate health, safety, or personal rights risk to persons in care. Director self reported and submitted a written Unusual Incident Report (UIR) to the department.
