What the inspector wrote

On April 11, 2023 an unannounced plan of correction inspection was conducted by Licensing Program Analysts (LPA's) Lea Habtom and Michelle Pascual. The purpose of today’s visit is to follow up on the deficiencies assessed on March 14, 2023. LPAs met with designated staff member Lucia Vargas. All staff present today have fingerprint clearances and associations. The census for today was 45 preschool children being supervised by 6 staff in classrooms 2's, 3's, 4's and TK. Based on today's census, LPA's cited for facility being out of ratio due to not having enough fully qualified staff to children in care. LPAs were unable to clear the deficiency cited for the complaint substantiated on March 14, 2023. LPAs cleared one deficiency based on observation. Cleared plan of correction letter was provided to designated staff Lucila Vargas. LPAs reviewed remaining deficiencies to clear and will make a return visit to correct the remaining deficiencies. In the areas that were evaluated, deficiencies were cited during today’s inspection on 809-D. Exit interview conducted and report was reviewed with the Lucila Vargas. A notice of site visit was given and must remain posted for 30 days. Appeal rights were provided. Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit
