What the inspector wrote

Licensing Program Analyst (LPA) Christopher Bello arrived at the facility at approximately 10:20am and met with Acting Director Lucia Vargas. During an interview it was revealed that the facility was left out of Teacher-Child ratio during an incident when as staff had to leave their class to prevent a child from leaving the facility. LPA made observations, interviewed staff and children. Director stated that she has spoken their District Leader and Inclusion Services to better help with children with behavioral issues. Title 22 Deficiencies have been cited on the attached LIC 809D . Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 809D in each child's files This report was reviewed and discussed with licensee. A notice of site visit and appeal rights were provided.

    A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children. This requirement has not been met by evidence: An interview revealed that a class was left out of ratio when a staff followed another child to prevent them from leaving the facility. This is considered as an immediate risk to the children in care.
