What the inspector wrote

On January 13, 2026, Licensing Program Manager (LPM) Mai Lor and Licensing Program Analysts (LPAs) Soleil Marx and Joshua Hatch, met with District Leader, Austin Othman and Director, Dawna Allred, for the purpose of conducting an informal office meeting. LPM defined the difference between a non-compliance conference and an informal meeting. LPM advised that the purpose of today’s informal meeting was to discuss supervision after an absence of supervision incident that occurred on 08/20/2025 and to discuss ratio/qualifications due to a ratio citation issued on 11/20/2025. The facility has implemented the following procedures to maintain compliance: - Facility conducted 90 days extensive CSR training/oversight of transitions - Facility implemented on-going pop-in management oversight - Facility has conducted re-training on attendance counts, name to face counts, sweeps - Facility has on boarding CSR training in place and conducts on-going training at regularly scheduled staff meetings - Facility has hired additional staff and reviewed ratio/qualification requirements LPM and LPA provided information regarding the Technical Support Program (TSP), which is a non-enforcement arm of the Community Care Licensing Division offering onsite support to licensees and providers.
