What the inspector wrote

On 10/23/2025, Licensing Program Analyst (LPA) Soleil Marx, met with Director, Dawna Allred, for the purpose of conducting an unannounced case management inspection. LPA observed a census of 24 infants being supervised by six staff within two classrooms. It was determined by record review that the facility had internally investigated a personal rights allegation and determined the personal rights violation to be substantiated. (see 9099 from 10/23/2025) The facility attempted to remediate the violation with coaching, however failed to report the unusual incident to the department by phone or written unusual incident report. A deficiency for being out of compliance with reporting requirements is being cited on the 809-D page. Exit interview conducted and report reviewed with Director. A notice of site visit was provided and must remain posted for 30 days. Appeal Rights were provided.
