Inspector’s narrative

What the inspector wrote

On December 7, 2022, at approximately 9:30 AM Licensing Program Analyst (LPA) Josiah Gathing met with Director Cheryl Watt to conduct an unannounced case management inspection. During today's inspection there were 15 preschool children being supervised by 4 teachers. The purpose of today's inspection was to create a plan of correction following reports of lead exceedance in three of the facility outlets. On 10/8/2022, the facility tested water samples for lead. Three water outlets sampled indicated an Action Level Exceedance (ALE). These outlets are marked H, I, and M on the facility sketch provided by the center. H correlates to a sink faucet in the staff area between the toddler rooms. I and M refer to sink faucets in the kitchen. On 11/11/2022 the facility was notified that water levels were in exceedance of 5 parts per billion in the locations that were tested. During today\342\200\231s inspection, LPA observed that outlets H, I, and M are inaccessible to preschool children. All faucets used by preschool children have been tested and were not in Action Level Exceedance. Additionally, signs have been placed on each outlet in exceedance stating that the outlets should not be used for drinking or food preparation. LPA received the lead testing map and other required documents from the Director during today's inspection. A deficiency is cited on the following LIC809-D and the plan of correction was reviewed with the Director, Cheryl Watt. A copy of this report and appeal rights were provided to the Director.
