What the inspector wrote

On January 26, 2023, at 11:15 AM, Licensing Program Analyst (LPA) Tanya Washington met with Facility Representative, Bhavneet Kaur for the purpose of an unannounced case management inspection. The purpose of the inspection is to address an unusual incident which occurred on 01/18/2023. Upon arrival LPA observed 11 staff providing care and supervision to fifty children in five different classrooms. On 01/18/2023, the facility self- reported an incident of absence of supervision. During today's inspection LPA conducted interviews with staff. During the interviews LPA learned that Staff #1 was in the bathroom with three children. While children were using the bathroom and being observed by Staff #1, Staff #2 was passing the bathroom area, returning fro m lunch. Staff #1 asked Staff #2 to take over the supervision of children in the bathroom so they can go to lunch. Staff # 2 acknowledged and a greed to take over. Staff #2 assisted two children back to their designated classroom, leaving Child #1 unattended in the restroom area. Staff #3 observed Child #1 standing in the doorway of the bathroom, they assisted Child #1 and brought them to Facility Representative. Based on the interviews, absence of supervision was approximately for one minute. Since the incident occurred, facility management has talked to the staff involved in the incident regarding supervision and safety of children. The facility management will be providing safety and supervision training on February 3rd, 2023. Staff has also been reminded to scan all areas during transition time and everyone is encouraged to contact management or Family Support Service employees if help is needed anytime. Upon receipt, facility representative shall post and provide copies of this licensing report to parents/ guardians of children who are currently enrolled as well as parents/ guardians of children newly enrolled at the facility during the next 12 months. Parents/guardians must acknowledge receipt of this report and citation by signing a LIC9224, \342\200\234ACKNOWLEDGEMENT OF RECEIPT OF LICENSING REPORTS\342\200\235. A copy of this form should be placed in each child file upon receipt from parent. Refer to LIC809D for citation information. LPA discussed this report with facility representative and conducted an exit interview. LPA also provided appeal rights. Notice of site visit posted.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidenced: Child #1 was left unattended in the bathroom for approximately one minute. This is an immidiate health and safety violation.
