On 12/16/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted a Case Management inspection to verify corrections of a deficiency cited on 11/20/2025. LPA arrived at the facility and was met by Licensee Donyale Roberts (D1). LPA disclosed the purpose of the inspection and was granted entrance into the Center. LPA observed a census of 34 preschool aged children. LPA determined, through accessing Guardian, that all required adults were background cleared and associated to the license. On 11/20/2025, the facility was cited a Type B deficiency for lack of complete staff files. LPA reviewed staff files during today's inspection. The Type B deficiency was cleared, and LPA provided D1 with Proof of Correction letter. No deficiencies were cited during today's inspection on 12/16/2025 in the areas that were evaluated. An exit interview was conducted, and the report was reviewed with Licensee Roberts. Licensee Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100.
