What the inspector wrote

On 05/05/2026, Licensing Program Analyst Katy Velazquez (LPA1) and Licensing Program Analyst Denice Pablico (LPA2) conducted a field visit to the facility for the purpose of an unannounced annual inspection. LPA1 disclosed the purpose of the inspection and was granted entrance into the facility. LPAs met with Director Cindy Sigala (D1). LPAs toured the facility inside and outside. LPAs observed 3 school aged children being supervised by 1 staff member. LPA1 accessed Guardian to verify that all required adults were background cleared and associated to the license. LPA1 assessed that 1 staff member had been removed from the license in January 2026. D1 confirmed that there are no Registered Sex Offenders employed at the facility. LPA1 completed the RSO profile on 05/04/2026. Facility days and hours of operation are Monday-Friday from 6:30 AM to 6:00 PM. The facility provides breakfast, lunch, and 1 snack per day. LPA1 conducted a file review prior to inspection and verified that annual fees were current. While on location, LPAs observed that hazardous items (disinfectants, cleaning solutions etc.) were inaccessible to children in care. LPAs reviewed care and supervision of children, staffing ratios, medications, first aid supplies, furniture, equipment, fire drills, and drinking water. LPAs observed all required forms to be posted. There appeared to be adequate toys and equipment available for children with available shade. LPAs toured the outdoor play area. D1 stated that the pay structure is not used. LPAs observed a cart for outside play with water, cups, and first aide supplies. LPAs reviewed the sign in/out documents and observed that the children who were currently in care were signed in. LPAs reviewed children’s and staff files. LPAs observed Health Screening Reports with TB test and required MMR and TDAP vaccines for all present staff members. At least one staff member present today had current Pediatric CPR and First Aid (exp. 09/2027). LPAs observed current Mandated Reporter training. D1 was reminded to renew the course every 2 years. Report continues on 809-C This training requirement may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com/ . The training is currently provided in English and Spanish. This facility provides Incidental Medical Services (IMS). LPAs reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred the facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES in June 2022. D1 was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA1 informed Director Sigala that this report, dated 05/05/2026, document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Velazquez informed L1 to provide a copy of this licensing report dated 05/05/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, and the report was reviewed with Director Sigala. LPA1 provided Licensee Appeal Rights to D1. A Notice of Site visit was posted by LPA1 and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
