What the inspector wrote

On Monday, 12 January 2026 at approximately 10:00am, Licensing Program Analyst (LPA) Fabian Schwartz met with Program Coordinator Tamara O'Callaghan for an unannounced, case management inspection to discuss the Unusual Incident Report (UIR) that the program submitted on 19 December 2025. There were no children present during the time of inspection because the 4th R Program operates after school hours. An unusual incident was reported to the department involving children attending the Two Rivers Elementary school. LPA interviewed staff and toured the facility during today's investigation. In the areas that were evaluated, no deficiencies were observed at the time of the visit. An exit interview was conducted and a Notice of Site Visit was posted.
