What the inspector wrote

Licensing Program Analyst (LPA) Loraine Perez met with Director Mary Patten (Director), for the purpose of conducting an unannounced investigation inspection to open and close a complaint pertaining to the above allegation. The purpose of today's inspection was explained to Director It is alleged that staff are commingling the toddler component children with the preschool children. Present today there were 13 staff providing supervision to 59 children throughout the preschool program. During today's inspection, LPA conducted interviews, observed care, and obtained relevant documentation. Witness statements, LPA observations, and document reviews failed to corroborate the allegation. At the time of inspection there were two fully qualified teachers and one aid supervising 14 children, which is within the ratio for the Toddler component. From statements of those pertinent to the investigation there was not confirmation that the center commingled children beyond what is permitted by the waiver to commingle the first and last hour of the hours of operation. Although the allegatio may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Director, Mary Patten. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.

    The person who signs the child in/out shall use his/her full legal signature and shall record the time of day.This requirement is not met as evidenced by: Based on interview and record reveiw, the licensee did not comply with the section cited above in that when the electronic system is not working there is not a paper option to collect signatures which poses a potential risk to the health, safety of persons in care.

    Solid waste shall be stored, located and disposed of in a manner that will not transmit communicable diseases or odors, create a nuisance, or provide a breeding place or food source for insects or rodents.This requirement is not met as evidenced by: Based on interview and observation, the licensee did not comply with the section cited above in an odor can be detected when entering the toddler classroom, which poses a potential risk the health and safety of persons in care.
