What the inspector wrote

An unannounced case management inspection of the facility was conducted by Licensing Program Analyst Jennifer Velasco (LPA). LPA met with facility Director owner Rebecca Michael (D1), toured the facility inside and outside, and reviewed facility records. The purpose of this case management inspection was to follow up on a self-reported incident in which an infant left a classroom and was immediately retrieved by a staff member. The facility provided an additional staff to that classroom to ensure adequate supervision was provided to prevent children from attempting to abscond. During this inspection, ten classroom staff were providing care to twenty-eight children, operating within ratio and capacity requirements. It was determined the facility was compliant with health and safety regulation as required. No deficiencies were cited during today's inspection. This report was reviewed with facility representative, D1. Notice of site visit was provided and must be posted for 30 days.

This requirement was not met as evidenced by witness statements, facility documents, and video footage that showed a staff (S1) handled a child (C1) roughly, causing C1 to impact the wall with their head and body and fall. This constitutes an immediate threat to the health and safety of children in care.
