What Happened
Annual inspection of multi-program facility with loose carpet hazard on transition stairs. Two staff members (Staff #4 and #6) were not fingerprint cleared and were required to leave facility. Two additional staff members missing current Mandated Reporter training.
Observations
- Loose carpet on stairs used for outdoor transitions identified as potential tripping hazard; scheduled for repair over weekend
