What Happened
Case management inspection for unusual incident found documented lack of supervision in outdoor play area. Inspector observed staff concentrated in one location unable to monitor entire yard, particularly during transitions and parent interactions.
Observations
- Lack of supervision in back end of preschool play yard near play structure
- Staff observed concentrated near sandbox area only
- Staff unable to maintain 100% supervision during transitions
- Staff unable to maintain 100% supervision during snack service
- Staff unable to maintain 100% supervision during sign-in and sign-out
- Staff unable to maintain supervision while communicating with parents

(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met, evidenced by: Based on observations and staff interviews, there is a lack of supervision during transistion periods, and staff are unable to provide 100% supervision.
