It was alleged facility staff covered child’s face with a blanket. During the investigation, LPAs conducted a observations, and interviewed reporting party, staff, children, and parents . After observations and interviews, LPAs did not learn of any evidence of staff members covering child(rens) faces with blankets. Although the allegations above may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Exit interview conducted and report reviewed with facility representative, director April Vasquez. Appeal rights provided. Notice of site visit was given and must remain posted for 30 days.
