What the inspector wrote

*Times on the report are approximate*. At 9:20 am on June 23, 2022, Licensing Program Analyst (LPA) Lea Habtom met with Director, Nova Williams, for the purpose of an unannounced required 1 year inspection. Operating hours of the facility are from 6:30 a.m.-5:30 p.m., Monday thru Friday. Director guided LPA on a tour of the facility, at which time a census of 13 preschool children supervised by 2 staff was observed. LPA was not able to confirm staff supervising children were fully qualified. All individuals subject to criminal background review have obtained criminal record clearance. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A health and safety inspection was conducted in the classrooms, restrooms, food service areas, and outdoor play areas. LPA observed the following documents are posted: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, menus, and daily schedule. Cleaning disinfectants and hazardous items are appropriately stored and inaccessible to children. Medications are stored, inaccessible to children. Director stated there are no poisons on the premises. Furniture and equipment are in good condition, and toileting facilities are in safe, sanitary, and operating condition. Bins for solid waste in the have tight fitting lids. The floors appeared clean throughout the facility. The facility provides breakfast and afternoon snack and parents provide lunch. The food preparation space is free of litter and all food was protected against contamination. Drinking water was readily available to children both indoors and outdoors via water jug and labeled bottles. Facility uses a paper sign in and sign out. LPA observed full legal signatures of authorized representatives. There are no firearms or bodies of water on the premises. LPA observed a functional carbon monoxide detector. Playground equipment and surfaces are free of loose or sharp. Report continued on 809-C parts. LPA observed artificial grass beneath the play structure. Outdoor shade is provided by trees and tents. Staff files were reviewed. LPA could not confirm one staff member present has a current CPR. LPA was unable to confirm immunization records for staff. LPA verified documentation of the educational background, training, and/or experience and AB 1207 Mandated Reporter training certificates. Children's records were reviewed. Each child's file contained an emergency card, consent for emergency medical treatment and notifications of children\342\200\231s and parent\342\200\231s rights, health history, physician's report and immunization records. LPA observed signed form LIC224 Acknowledgement of receipt of licensing reports. This facility does not provides Incidental Medical Services \342\200\223 IMS. A plan of operation is on file at the facility. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s personnel and administrative records. LPA verified the annual fees are current. A staff interview was conducted with the Director, Nova Williams. In the areas that were evaluated, deficiencies were cited during today\342\200\231s inspection. See 809-D for deficiencies. Exit interview conducted and report was reviewed with Director, Nova Williams. A notice of site visit was given and must remain posted for 30 days. LPA cleared the deficiency that staff were supervising children without being fully qualified by having the director in ratio. \302\267 Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type \342\200\234A\342\200\235 deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child\342\200\231s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.
