At 11:13 a.m. on Monday, August 2nd, 2021, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Cindy Jones, for a case management inspection. A COVID-19 risk assessment was conducted prior to entering the facility. At 11:15 a.m. Director guided LPA on a tour of the facility, and a census was taken which included 4 infants supervised by 1 staff, and 17 toddlers supervised by 5 staff. Census for the preschool facility was 45 children supervised by 5 staff. Today's case management inspection was conducted to follow up on an incident that occurred at the facility on July 14th, 2021. Director self-reported the incident to the department on July 15th, 2021. LPA viewed video surveillance, gathered documents, and conducted interviews. No deficiencies were cited during today's inspection. An exit interview was conducted. This report was reviewed with the Director and a notice of site visit was provided. Notice of site visit shall remain posted at the facility for a period of 30 days for parental review.
