What the inspector wrote

On 12/07/2023 at approximately 9:10am, Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Arianna Manabat and Licensing Program Analyst (LPA) Lea Habtom, and Licensing Program Manager (LPM) Mai Lor met with Licensee Representative Mikako Fisher Hill for the purpose of adding Toddler Option component to the preschool license case management inspection. Licensee requests to add 16 toddler children ages 18-36 months to the preschool license with a total capacity of 40 (24 preschool children and 16 toddler option children). The fire clearance was received and granted on 07/27/2023. The program operates Monday through Friday from 8:00AM - 2:30PM. INDOOR ACTIVITY SPACE: There are two two buildings on site that are used by the facility. The toddler classroom is located in Room #19 and the preschool classroom is located in Room #20. CAB LPA observed a sufficient amount of equipment, toys, tables, chairs, nap mats, and cubbies. There are no changes to the facility's indoor and outdoor space; therefore, CAB LPA used measurements from report dated 11/05/2021. The total indoor space contains a total of 2,110.57 square feet, which will accommodate Licensee\342\200\231s request for 24 preschool children and 16 toddlers in the toddler option program. There are a total of five sinks, which two sinks are portable, and four toilets. CAB LPA advised the Facility Representative section 101239(e)(4) Fixtures, Furniture, Equipment and Supplies: All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition. There is a private restroom for staff located on site. OUTDOOR ACTIVITY SPACE: There is one preschool outdoor area on the property. The Facility Representative stated the toddler option children will be sharing the outdoor activity space. Facility Representative was advised to submit a shared outdoor waiver request with a rotating schedule of the times when each group of children will be utilizing the outdoor space. The outdoor play area is fenced with a chain link fence that is at least four feet tall. CAB LPA observed a sufficient amount of equipment and toys available for use. Report continues on LIC809-C.... There is a play structure that is designated for children ages 2 to 5 years. Facility Representative was reminded to follow the safety label and staff will provide supervision to ensure children under the age of 2 will not be using the play structure. There is a second play structure without a safety label. The second play structure does not have a safety label to include the ages suited for. The Facility Representative was reminded to follow manufacturer guidelines. Facility Representative stated there are no firearms, weapons, or bodies of water on the facility premises. There are shaded areas supplied by trees and overhangs. There are no changes to the outdoor activity space; therefore, CAB LPA did not take measurements. CAB LPA used measurements from report dated 11/05/2021. The outdoor play area contains a total of 7034.61 square feet, which will accommodate Licensee\342\200\231s request for 24 preschool children and 16 toddler children. Licensee Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Prior to issuing a license, the following conditions must be submitted: 1. Shared outdoor waiver request for the toddler option children to share the preschool play yard. 2. Pending Licensing Program Manager's final review. Exit interview conducted and report was reviewed with the Licensee Representative, Mikako Fisher Hill.
